Annual Action Plan for Student Success Program (Grade 7-12)


Overview

Beginning this year, Ontario district school boards and school area boards are required to generate student information on 7 "Key indicators of Success" as described by the Ministry.  These indicators have been identified through studies on the double cohort and previous results on the Literacy component required for graduation. 

The Annual Action Plan report option in Maplewood will generate information required for the 7 "Key indicators of Success" area that will assist you in completing the report in its entirety.

All credits achieved are included in indicator 1. Previously only courses that were completed in the school year were included.

By definition:
Method 1 - Includes all Eligible First Time literacy test writers 

Method 2 - Those eligible first time literacy test writers who actually wrote the test. 
(This will include students who have Not Applicable and a Non-Participation Status of Deferred or Exempt)

- Students who have Not Applicable, without a Non-Participation Status are NOT included in either method nor in the report as this data is inaccurate (incomplete).


Indicators 4 & 5 

- For the 'Multi' type courses (Multi-section, Multi-grade, etc), the program looks for the same teacher, schedule and room. The actual Maplewood Grouping flag is not regarded.

Indicator 7 

- Regarding Coop Courses, the report only includes the Guidance and Career Education Coops sited on the report. The program looks for the course code. 

Indicator 8

- We are able to provide the numbers for the first column, "Day Students Registered on Oct. 31/04. It is likely, we have been told, that the schools will have to run the report again in October to have the other two columns populated as the required statistics are not available at this time.

Dual Credit/SHSM Report and IEP - The program includes students under the IEP row if the student meets the following criteria:

-Student has Special Ed - Yes (enrolment tab) Active (Spec Ed Tab) Placement and Placement Date (Spec. Ed Tab) Exceptionality is no longer a requirement.

Choose File, Student, student records, Transcript, Edit.

1. Total number of students who earned 8 credits (successfully).

Total # of students in grade 9 (of the reporting year).

Note: # of credits for each grade will only include courses of that grade. If a student in a particular grade is taking courses in another grade then the course will be counted in the other grade.

Pass Rates for Compulsory Credits (Grade 9 & 10)
Students included:
Pass = all students who took one of the respective courses (i.e. ENG1D*).

Total Students - the number of students registered in the course (Including students repeating the course and those who have withdrawn from the courses after the first reporting period - i.e. recorded withdraw within the student's transcript).
Total number of students who passed course.

The calculation of literacy test is based on the following:

  1. The student's entry date to this school must be before the report end date.

  2. The student must be in grade 10 (in the reporting year).

  3. The literacy information must be English, French, Incomplete.

2. Total number of students with a passing mark in OLC40, OLC4O, CCL40, CCL40.

Annual School Leaver Rate
All students with a retirement date between Nov 1, 2003 - October 31-2004 who have a Destination Type with a MIN code of 11,15, 16, 17, 25, 26 (or Maplewood defined Unknown).

Total Enrolment on October 31, of the previous year (Section K of October Report for each school in Board).
 

 

Creating the Report

At the school level:

  1. Choose Reports, Annual Action Plan.

  2. Select the appropriate start and end dates for the indicated school years, then choose the option to Create the file.

  3. Once the file has been created, select View.

  4. The ability to save the report in PDF format is available when view the report. Select the arrow beside the print button, a drop down menu will appear, select the option to Save to PDF.  Enter the name that you would like the report to be saved as (there is not a default name for the report), then select Save.

  5. A list of all students included in each sections can be accessed by selecting the Details option. From the pull-down menus, select the Year, Section and Details (within that section) that you would like to see. The information will automatically be displayed. To save (or print the report), select the Print option. Follow Step 4  instructions to save the file.

From Maplewood Central Office:

  1. Choose Reports, Annual Action Plan.

  2. Select the Schools button, and select the Secondary schools that should be included in the report. Note: Elementary schools should not be selected as the report information only includes secondary information. Then select OK.

  3. Select the appropriate start and end dates for the indicated school years, then choose the option to Create the file.

  4. Once the file has been created, select View.

  5. The ability to save the report in PDF format is available when viewing the report. Select the arrow beside the print button, a drop down menu will appear, select the option to Save to PDF.  Enter the name that you would like the report to be saved as (there is not a default name for the report), then select Save.

  6. A list of all students included in each sections can be accessed by selecting the Details option. From the drop-down menus, select the Year, Section and Details (within that section) that you would like to see. The information will automatically be displayed. To save (or print the report), select the Print option. Follow Step 5  instructions to save the file.

Note: Central Office must have a calendar set up (as the report relies on calendar dates). If calendar does not exist, an "error loading form" when opening the Annual Action Plan will occur.
